Accident Summary:
1LT HICKMAN REFUELED HIS AIRCRAFT AT ONE OF THE REFUELING POINTS AT CHU LAI EAST ARMY AIRFIELD. IN ORDER TO ALLOW THE SECOND OH-6A TO REFUEL, HE REPOSITIONED HIS AIRCRAFT TO THE NEXT REFUELING POINT. AS HE BEGAN A TURN TO THE LEFT, A HUEY HOVERED BY, CAUSING THE AIRCRAFT TO MOVE REARWARD UNTIL STRIKING A MTEAL POST USED TO SUPPORT A FIRE EXTINGUISHER. THE PILOT WAS AT AN APPROXIMATE 3 TO 5 INCH HOVER AND IMMEDIATELY LOWERED THE COLLECTIVE. THE AIRCRAFT CAME TO REST ON THE MUDDY PAD WITHOUT ANY FURTHER DAMAGE AND WITHOUT A HARD LANDING. AS THE AIRCRAFT DRIFTED REARWARD, THE TAIL ROTOR BLADES STRUCK A METAL POLE CAUSING THE TAIL ROTOR BLADES TO DISINTEGRATE. IT ALSO CAUSED SUDDEN STOPPAGE TO THE TAIL ROTOR GEAR BOX, THE TAIL ROTOR DRIVE SHAFT AND THE MAIN TRANSMISSION. AS THE TAIL ROTOR BLADES FLEW APART, THE PIECES WERE HURLED AGAINST THE AIR INTAKE COWLING, CAUSING LACERATION IN THE FIBERGLASS SKIN OF THE COWLING. THE TAIL ROTOR DIIVE SHAFT SNAPPED APPROXIMATELY THREE FEET FORWARD OF TAIL ROTOR GEAR BOX.\\

Accident Summary:
ON 4 JULY 1971 AT APPROXIMATELY 1000 HOURS THE PILOT DEPARTED TIEN PHOUC IN ORDER TO PERFORM A VISUAL RECONNAISSANCE IN THE VICINITY OF LZ CHIPPEWA. HE HAD REFUELED AT TIEN PHOUC. AT APPROXIMATELY 1030 HOURS HE DEPARTED FOR LZ BOXER TO CONTINUE THE VISUAL RECONNAISSANCE. AT APPROXIMATELY 1045 HOURS HE DEPARTED FOR TRA BONG IN ORDER TO REFUEL. WHEN HE WAS APPROXIMATELY ONE MILE WEST OF TRA BONG, PASSING THROUGH 1800 FEET IN A SLOW DESCENT AND APPROXIMATELY 90 KNOTS IAS, THE ENGINE SUDDENLY FAILED. NEITHER THE PILOT NOR ANYONE ON BOARD HAD ANY PRIOR INDICATION THAT IT WOULD DO SO. THE PILOT ENTERED AUTOROTATION AND CHOSE A TOUCHDOWN AREA. THE AREA WAS A LARGE SAND BAR IN THE RIVER BELOW. AS THE AIRCRAFT DESCENDED THROUGH 75 FEET THE PILOT FELT HE WOULD HAVE NO TROUBLE MAKING HIS AREA AND BEGAN TO FLARE. AT APPROXIMATELY 15 FEET HE MADE HIS INITIAL PITCH PULL. HE REALIZED MORE PITCH WOULD BE NEEDED TO STOP THE AIRCRAFT'S FORWARD MOTION, APPLIED ADDITIONAL PITCH AND ATTEMPTED TO LEVEL THE AIRCRAFT. HE THEN APPLIED THE REMAINING PITCH IN AN EFFORT TO CUSHION THE LANDING. THE AIRCRAFT LANDED HARD IN A TAIL LOW ATTITUDE CAUSING A MAIN ROTOR BLADE TO FLEX DOWN AND SEVER THE TAILBOOM. AFTER THE AIRCRAFT CAME TO A REST, EVERYONE ON BOARD EXITED WITH NEGATIVE INJURIES.\\
