Accident Summary:
THE PILOT HAD JUST RETURNED FROM R & R THE PREVIOUS DAY AND ARRIVED AT THE FLIGHT LINE AT 0900 HOURS, 30 JUNE 1970. AFTER BEING BRIEFED ON A VISUAL RECONNAISANCE MISSION HE WAS TO FLY, HE PROCEEDED TO THE AIRCRAFT, OH-6A, SN 68-17167. AFTER MAKING A PREFLIGHT HE STARTED THE HELICOPTER AND NOTICED HE HAD THREE HUNDRED POUNDS OF FUEL REGISTERING ON THE FUEL GAGE. HE TOOK OFF AND FLEW TO THE VICINITY WEST OF HAU DUC TO CONDUCT HIS MISSION. AFTER FORTY MINUTES OF FLIGHT HE NOTICED THAT THE FUEL GAGE WAS NOW REGISTERING 150 POUNDS OF FUEL. AFTER FORTY-FIVE MINUTES OF FLIGHT IT HAD DROPPED TO 100 POUNDS OF FUEL. AT THIS TIME THE PILOT ELECTED TO BREAK STATION AND RETURN TO CHU LAI EST, HIS HOME STATION. AFTER ENTERING AN EXTENDED RIGHT BASE INTO THE TRAFFIC PATTERN AT CHU LAI EAST, THE PILOT WAS AT 150 FEET ABOVE GROUND LEVEL AND APPROXIMATELY SEVENTY KNOTS AIRSPEED INDICATED. ON SHORT RIGHT BASE THE AIRCRAFT EXPERIENCED A LOSS OF POWER, THE ENGINE SURGED AND THEN QUIT. THE MASTER CAUTION LIGHT CAME ON DUE TO LOW RPM. AFTER THE SURGE OF POWER THE ENGINE QUIT AND THE "ENGINE OUT" LIGHT CAME ON. IMMEDIATELY THE PILOT ENTERED AUTOROTATION AT 130 FEET ABOVE GROUND LEVEL AND SEVENTY KNOTS AIRSPEED INDICATED WITH THE ROTOR RPM IN THE LOW GREEN. DURING THE AUTOROTATION THE PILOT STATED HE WAS IN A NORMAL DESCENT AND PICKED OUT THE INTENDED POINT OF TOUCHDOWN. WHEN NEARING HIS INTENDED TOUCHDOWN POINT AT APPROXIMATELY THIRTY FEET ABOVE GROUND LEVEL, THE PILOT NOTICED A SMALL KNOLL AND ELECTED TO EXTEND HIS GLIDE OVER THE KNOLL BY APPLICATION OF PITCH. THE PILOT DID NOT RECALL AT WHAT ALTITUDE OR WHEN HE FLARED. THE AIRCRAFT MADE CONTACT WITH THE GROUND WITH A HIGH RATE OF DESCENT DUE TO THE IMPRESSIONS LEFT IN THE GROUND FROM THE SKIDS. UPON CONTACT WITH THE GROUND THE TAIL BOOM ALSO STRUCK THE SMALL KNOLL AND WAS TORN FROM THE AIRCRAFT. SIMULTANEOUSLY THE BLADES FLEXED AND CONTACTED THE REAR OF THE AIRCRAFT IN THE VICINITY OF THE FAIRING ASSEMBLY. THE AIRCRAFT SKIDDED FORWARD APPROXIMATELY FIFTEEN FEET, SPUN 110 DEGREES TO THE LEFT AND CAME TO REST. THE TAIL BOOM, TAIL ROTOR ASSEMBLY, AND PART OF THE ENGINE FAIRING ASSEMBLY WERE APPROXIMATELY THIRTY TO FORTY FEET TO THE REAR OF THE AIRCRAFT. WHEN THE AIRCRAFT CAME TO REST THE PILOT AND CREW EXITED IMMEDIATELY WITHOUT INJURY. THE FLIGHT HAD TAKEN ONE HOUR AND FIFTEEN MINUTES. THERE WAS NO CRASH FIRE.\\
