Accident Summary:
CPT Elder was scheduled to perform an aerial visual reconnasious mission at 0830 on 20 July 68. He arrived on the flight line at 0800 in order to preflight his aircraft. The initial preflight was normal with no adverse write-ups on the 2408-13. This aircraft was just released for flight from field maintenance. During the preflight, it was discovered that the engine had been serviced with the wrong type of engine oil. The crew chief drained the engine oil and reserviced the aircraft. After a thirty minute delay the aircraft was mission ready. The run-up was normal with all instruments apparently functioning normally. The fuel quantity gage showed approximately 200 pounds of fuel on board. During the flight period, everything was normal with no caution or warning light illuminated. The first indication of the engine failure was a distinct power loss and the activation of the "engine out" warning system. At this time, the aircraft was approximately 15 feet and 30 knots and over a dense hedgerow. In order to avoid crashing into the hedgerow, CPT Elder attempted to zero out his airspeed and initiate a hovering autorotation. However, because of the induced flare, the helicopter entered a rice paddy tail low. When the tail rotor hit the water in the paddy the ninety degree gear box was sheared. The resulting forces caused the main rotor to sever the tail boom. The helicopter then settled in three feet of water without further damage

Accident Summary:
THE AIRCRAFT WAS ON A COMBAT SUPPORT MISSION, DEPARTED QUANG TRI AIRFIELD WITH ONE PASSENGER AT APPROXIMATELY 1040 AND AT 1042 LANDED AT LZ CABRA. UPON LANDING THE PASSENGER WAS DROPPED OFF AND THE PILT REDUCED THE ENGINE TO GROUND IDLE AND PUT FULL FRICTION ON THE CYCLIC AND ABOUT ONE HALF FRICTION ON THE COLLECTIVE. AFTER WAITING SEVERAL MINUTES FOR THE OTHER PASSENGERS THEY APPROACHED THE AIRCRAFT AND PROCEEDED TO BUCKLE IN. SHORTLY AFTER THEIR ENTRY THE AIRCRAFT SUDDENLY BECAME AIRBORNE AT A HOVER, ASSUMED A SLIGHT NOSE LOW ATTITUDE THEN SETTLED DOWN TO THE PAD WITH A NOSE HIGH ATTITUDE HITTING THE PAD HARD.\\
